6.08.030 License - Application.
(1) An application for a business license to conduct a towing or parking enforcement operation shall be filed in writing with the Provo City Business License Office as set forth in Section 6.01.060, Provo City Code. In addition to the information required by such section, the license application shall include:
(a) Identification of vehicle operators employed by the business;
(b) A statement disclosing whether any person listed in the application or any driver employed by the business has ever been convicted of a felony or misdemeanor, and if so, the nature of the offense, and where and when it was committed; and
(c) A description, vehicle identification number, and license plate number of each tow truck and parking enforcement vehicle used in the business.
(2) If any of the foregoing information changes, the business shall deliver current information to the Provo City Business License Office within ten (10) business days following the change.
