(1) evidence of a valid business license issued by Provo City;

(2) a non-transferable taxicab business license sticker issued by the license coordinator for each vehicle licensed under the holder’s business license. Stickers shall be displayed in the lower left portion of the rear window of each taxicab vehicle, extending no more than three (3) inches to the right of the left edge or more than four (4) inches above the bottom edge of the window; and

(3) a company identification card giving the name, address, and phone number of the company and identifying the driver as an employee thereof.
