It shall be unlawful for any employment agent, employee or agent, to furnish any worker to any employer of labor without having a bona fide order for the same from said employer and without having first secured from the employer reasonably full information concerning the prospective employment on a form prescribed by the state industrial commission, signed by the employment agent or the employment agent’s employee or agent, and by the prospective employee receiving the same. A copy of the said ticket shall be retained by the employee, one (1) by the employer, and a full copy of the same shall be retained by the agent and shall show the amount of commission or fees paid by the employee to the employment agent.
