6.28.060 Permit Application.
(1) An application for a newsrack permit shall be filed with the Business License Office upon a form provided by the City.
(2) The form shall require at least the following information:
(a) the name, address, and telephone number of the applicant;
(b) the name, address, and telephone number of a responsible person whom the City may notify or contact at any time concerning the applicant’s newsracks;
(c) evidence of the applicant’s qualification to do business in the State of Utah;
(d) evidence of the applicant’s Provo City business license;
(e) the name of the materials to be distributed by the newsrack;
(f) the number and location of newsracks on City-owned property or publicly-owned sidewalks for which the applicant desires to obtain a license;
(g) evidence of insurance required by Section 6.28.130 of this Chapter; and
(h) a site plan showing the location of each requested newsrack for which a permit is sought.
