An application for a business license shall conform to the requirements of Section 6.01.060 of this Title and shall include the following additional information:

(1) the address of each mobile home park owned, operated, or maintained by the applicant within Provo City;

(2) the owner or renter occupancy status of each mobile home within the mobile home park;

(3) the number of parking spaces provided within the mobile home park;

(4) if the owner of the mobile home park(s) is not a Utah resident, the name, address, and both home and business telephone numbers of a legal representative and agent who resides in the State of Utah for service of process;

(5) the name, address, and both home and business telephone numbers of a local agent who:

(a) resides not more than twenty (20) miles from the mobile home park(s), and

(b) is authorized to manage the mobile home park(s);

(6) proof of liability insurance for any mobile home park to be licensed; and

(7) the signature of the owner of the mobile home park(s):

(a) certifying, to the best of owner’s knowledge or belief, that the use and occupancy of the mobile home park(s) conforms to applicable ordinances, and

(b) agreeing to comply with applicable ordinances.

(8) A site plan, drawn to a recognized scale, which shows structures and mobile home spaces located on the property.
