(1) Certification of Permit Applications and User Reports . The following certification statement is required to be signed and submitted by users submitting permit applications in accordance with Section 10.04.190, Provo City Code; users submitting baseline monitoring reports under Section 10.04.260(2)(e), Provo City Code; users submitting reports on compliance with the categorical pretreatment standard deadlines under Section 10.04.280, Provo City Code; and users submitting periodic compliance reports required by Sections 10.04.290(1) through (4) , Provo City Code. The following certification statement must be signed by an authorized representative as defined in Section 10.04.040, Provo City Code:

I certify under penalty of law that this document and all attachments were prepared under my direction or supervision in accordance with a system designed to assure that qualified personnel properly gather and evaluate the information submitted. Based on my inquiry of the person or persons who manage the system, or those persons directly responsible for gathering the information, the information submitted is, to the best of my knowledge and belief, true, accurate, and complete. I am aware that there are significant penalties for submitting false information, including the possibility of fine and/or imprisonment for knowing violations.
