(1) Whenever a reporting entity makes any expenditure for an advertisement regarding a candidate or political issue that is broadcast or published by means of the internet, television, radio, newspaper, billboards, direct mailings, automatic telephone equipment, paid telephonists, leaflets, fliers, posters, bumper stickers or other printed matter, the advertisement shall disclose the name of the candidate or reporting entity who is responsible for its broadcast or publication.

(2) Whenever any person who is not a reporting entity, or acting on behalf of a reporting entity, makes an expenditure of more than five hundred dollars ($500.00) for an advertisement regarding a candidate or political issue which is broadcast or published by means of the internet, television, radio, newspaper, billboards, direct mailings, automatic telephone equipment, paid telephonists, leaflets, fliers, posters, bumper stickers or other printed matter, the advertisement shall contain the name of the person responsible for its broadcast or publication.
