(1) A valid Utah driver’s license which allows for operation of a tow truck or parking enforcement vehicle, as the case may be;

(2) Evidence of a valid business license issued by Provo City, if a license is required pursuant to Section 6.08.020, Provo City Code;

(3) In the case of tow truck operators only, evidence of a personal certification card issued by the Utah Department of Transportation; and

(4) A business identification card giving the name, address and phone number of the business and identifying the operator as an employee thereof.
