6.16.040 Application for License - Nature of License.
Licenses issued shall be personal in nature and each individual employee of corporations or other firms, shall be required to have a separate license. Such license shall be issued upon the filing with the City license department of a verified application upon a form to be prescribed by the licensing department setting forth the following details:
(1) The name under which the locksmithing is to be done.
(2) Name, address, and phone number of the applicant.
(3) Names and addresses of five (5) character references.
(4) A photograph taken within the preceding six (6) months.
(5) Fingerprints to be filed with the Provo Police Department.
(6) Statement that the applicant has never been convicted of a felony or a misdemeanor involving the property of others.
(7) Length of residence of the applicant in Provo City, and age of the applicant.
